It's a U. henkeli. I have it incubating on a small piece of foam in a ice tray which sits on top of a bed of moist vermiculite in a Rubbermaid shoebox. This is Kelly Sharp's incubation procedure, and she has had very good luck with it, so I am basically just copying it.
