It is a map turtle though I'm bad at telling the exact species apart. Where was it found? Knowing where the animal came from helps a lot with differentiating between similar species though I'm sure someone can tell you what king of map turtle it is without that info.

Yeah, probably a malformation of some sort. Not normal but if it's still alive and has the ability to eat then great.
