Green Water Dragons and Chinese Water Dragons are one in the same. Its hard to tell from the photo but it looks like there might be some calcium issues as well.

CWD tend to not do well in all glass aquariums, banging their heads regularly and smashing up their noses. When I have them here, I cover 3 of the 4 sides completely and also cover the first few inches of the front with cardboard.

It looks mostly rub, which you can treat with a triple antibiotic like neosporin, but I would personally look at getting into a vet for a full check up if the previous care was not so great.
