An adult doesn't need vitamins more often than once every 2 weeks. Possibly a slow overdose is affecting his vision or something else. What vitamin are you using? Calcium still about once a week, but these also depend on how well you are gutloading his feeders.

5 is getting up there. He's definitely a senior cham and may just be losing some of his grip, eye/foot coordination, possibly some joint problems or even loss of vision due to age. If he is less active he won't be wearing down the tips of his claws as well. Maybe some rougher barked branches and perches will help him grip more easily and wear the tips down more. If you clip them they won't necessarily grow back and may make falls more likely. You could put a denser network of perches in the cage to give him more gripping surfaces and help him break a fall.
