You need to get it tucked back in and have it stay in and possibly a vet visit. If it stays out and gets banged up, it'll get swollen and eventually infected and can kill them. If it's just hanging out for a little while as the lizard is underwater but goes back in once he's out, it's probably ok. Water does tend to cause them to drop a deuce and their units will protrude while pushing it out, sometimes remaining out for a little bit.
