I wonder if supersaturated (concentrated amounts of oxygen) gases; found in our tap water could be the cause of the problem.

Perform a water change, but, before you add fresh water to the tank work it over good, by pouring into one container from other; this breaks up the gas bubbles.

I think you'll see an improvement after a water change or two.

Increased surface action eliminates these bubbles as well.
