Help me from killing my horse!!!!!

Ok, what seemed to be a nice trip to the vet quickly became a horriable experience. He did not want to reload after the vet did his yearly checkup. Then when I got home, started to unload hin (by myself) he darted out before he was untied, broke the lead rope and gashed his head. I have trailered him a few times in the same trailer. I know how to work with him to load, but what can I do by myself to teach him how to back out like a calm horse instead of a maniac? Luckly I do put a helmet on him, but this really frustrated me. This was my first time solo. I did not want to untie him before the back door was open, because I was afraid of him taking off, so I opened the door first then started to untie him. Any suggestions to make mine and his life easier? Thanks.
