You are already doing so many things right. You just have a very aggressive baby on your hands.

One thing that might help are very intensive interactive playtimes with a feathered cat wand toy. These are said to defuse a lot of aggresive energy. Drag the toy away from your feet.

Since the squirt bottle doesn't seem to help, try a shake can - a soda can with some pennies inside - when the kitten does undesirable behaviors like attacking the dog. Then offer the kitten the alternative cat toy wand.

When he acts aggressively to you continue to put the kitten in "time out" - maybe you can kitten proof your bathroom and give him a little more space. Let him yowl like you've been doing and don't let him out until he quiets down. Reward him then with some play.

Another kitten would help, but you might not want that extra responsibility. Believe it or not two kittens cause less trouble than one most of the time because they play with each other! Here is an article on the web that might help:
