There is something seriously wrong with this cat. He should be taken to a vet for an exam - with the warning of how vicious he is. The vet may be able to determine a cause, or recommend an animal behaviorist or medication to calm him down while whatever is going on can be worked out.

The cat is not "evil" - he is responding to some unknown problem in an aggressive way. Unfortunately inbreeding may have caused this temprament. It's hard to determine. A vet is your grandmother's next best step. In the meantime the cat does need to be isolated when strangers are around.
