You have done everything right so far and your vet is actively involved!

A blood test could help screen out kidney failure or diabetes which have been known to cause problems with litter box usage.

However, most likely the problem is behavioral. Since you have tried other resources I would definitely ask the vet about anti-anxiety meds - particularly Buspar - in this case. Buspar is reputed to really help with litterbox issues and if issued under the guidance of a vet can be used and then gradually withdrawn as the problem gets under control.

Also ask the vet about getting in touch with a certified Animal Behaviorist.
