It sounds like you need to get a second opinion from a feline-only vet if possible. This is not to say that your vet isn't competent - very often one vet will experience one illness and learn from it while the next will have a different area of expertise.

I assume that the teeth have been checked, and that the mouth has been checked for tumors. Have you talked to the vet about stomatitis? Although it may not cause swelling like you describe, I am wondering if this is a possibility.
