Unusual elimination behavior

My 4 year old male cat, 1 of 3 who live together, is not using the litter boxes for urinating and pooping. I have observed him and he will become very panicked and begin running from room to room and leaving bits of poop as he runs. It is as if he is spooked by the urge to poop. This is so unusual. Afterwards, he is fine. He has been checked for urinary infections and he has none. The vet is prescribing Clomicalm in a 3 mg. tablet which I will be picking up in a day or so. The litter boxes are kept clean and I use the same litter as always. Has anyone had this experience with a cat doing this or using Clomicalm? I am wondering if he should have a blood test or an MRI. I would appreciate any suggestions. Thank you
