When a cat stops using the litterbox it's always best to start with an exam by a vet, and possibly labwork. You need to be sure that there isn't a medical reason for the inappropriate elimination first, then deal with the behavioral issues if that is the problem. The reason that the cat keeps going back to the same spot is because the smell is there. You need to use an enzymatic cleaner on the carpet to have any chance of removing the odor.

It's good that the younger cat is seeing the vet because there are many things that can cause vomiting, and some of these are quite serious. It can be as simple as the food not agreeing with the cat, or as complicated as liver disease. BTW, it's better for cats to eat canned food because it's closer to natural for them. Many cats have problems digesting dry foods because they aren't able to process the carbs in them. Some of the better quality dry foods contain no carbs, or very few, and these are healthier diets.

I recommend adding one more litterbox because ideally you should have one for each cat plus one extra one. Some cats are very picky about using a box that hasn't been cleaned after being used by another cat. It's best to use litter that isn't scented and best to stick to whichever one the cats prefer. Changing the type of litter will cause some cats to not use the litterboxes. Also, please be sure that the boxes are in quiet areas where the cats will not be startled, and don't move them around. Cats are creatures of habit.
