nasal discharge

my cat is 19 and having all sorts of issues. I've taken him to vet and bloodwork indicates his kidneys not up to par, due to age. He drinks lots of water, mainly in morning hours. He is eating real well but gaining no weight and has lost weight. He had a nasal discharge and vet gave Cefa Drops and while I thought it helped, the discharge is back and mainly from one nostril. The discharge is green and thick. His eyes are clear. He also developed some skin condition where his fur has thinned out and certain spots very crusty. Vet gave a shampoo for him and I've used it a few times, but condition persists. any suggestions on the nasal discharge. thank you.
