vet wants to extract teeth

I have a 5 yr old ex-feral cat who has had severe chronic gingivitis, unresponsive to multiple antibiotics and steroids. The vet now recommends teeth extraction . He drew a cbc and electrolyte panel to see if he could identify any underlying illnesses causing a immune response. He did test neg for the tests done on kittens. HE had a nasal polyp removed at 1 year and has always been a undernourished thin cat despite ample food. He does eat but in small amounts. I try to apply a dental gel several times a week. I have noticed his skin is very dry with alot of dandruff like flakes. The vet said this couild be from stress. Any imput will be appreciated.
