Some questions... are you new to tortoises? Describe your setup, hidespots, diet, appetite, temperatures, humidity, stool consistency ect

With any new reptile a vet visit is a good idea; at least for a stool sample if nothing appears wrong. A vet that is already familiar with tortoises should be able to compare yours to what is normal, give a thorough exam, and suggest any diagnostics that may reveal what is happening inside your tortoise. The description of a lethargic tortoise can mean lots, as you mentioned it may be as simple as heat avoidance. I can tell you that even on the hottest days (although I am in Canada )my russians still head toward the food dish whenever it's filled.
