I would recomend taking a fecal sample in to the vet if you suspect internal parasites. Also I would toss the paper roll and get him a nice rock hide or something he is totally covered in over the hotspot! Also a wet hide for when he feels he needs some more moisture for shedding or what not and maybe something on the cool end for him to hide in. Might make him feel a lil more at home and up his appetite.
Another thing i was going to say is that maybe he is just prefering the mealworms but depending on how many you are offering that still does not explain the drop in weight!
Like you said man your not new to this but just some things that would help out IMO as far as comfort.
