I'm going to need to see some more pictures...just kidding! Yes, it could be shell rot. Three-toes are very succeptible to shell rot. With the trauma she's obviously endured, it could have opened her up to an infection. Or, it could just be scarring. You should take her to a good exotic vet that is knowledgeable about turtles.
