I had the same problem until I got a hold of the account rep that handles my area. He set up my account over the phone overnighted me the info for the snake and walked me through step by step. The paper work I had to sign was:



"Waiver of Liability for Reptile and Amphibian Domestic Shipments"



It took about two-three weeks and everything was set.



If you have any problems shoot me an e-mail and I will give you my account reps number, I'm sure he can get you in touch with the proper person for your area.
