My bottle of Chlorhexidine says, "dilute 1 ounce per gallon of clean water."

Mine is Chlorhexidine Solution. Active ingredients: Chlorhexidine Gluconate 2%.

So, depending on what Chlorhexidine Solution you're using, 1 once per gallon of water is the proper way to do it.
