As far as holes are concerned - they really aren't necessary - if they go through the side foam they not only reduce the insulating properties but they also compromise the strength of the package.

Boxes aren't air tight by any means and as long as you aren't taping up all the seams you should get plenty of air exchange. If you absolutely have to have holes - it's best to just clip a small triangle off of one of the corners on your top piece of foam.
