Make arrangements to pick it up the day before the appointment so you can give it to her an hour or so before you leave the house. My vet does this, since my cat is agressive at the vets, but I don't know what it's called. Sure does do the trick though!

Oh geez, I agree with Destany. First it sounds like she could do with some anti-anxiety medication like Buspar or Prozac and secondly...what a terrible vet. Try taking her to a vet that will try to help her. Good luck with her. Please don't put her down.
