I'd start by looking at basic husbandry issues. What is his basking surface temp? Cool side? He should be eating way more than that. Have you had a fecal done to rule out parasites? You said that you don't give multivit, do you at least give calcium supplement? If he hasn't already, I'd take him in to the vet for a checkup and fecal, something sounds wrong.
