From your description of your supplementation regime I bet that the animal receives "enough" Ca and vitamines but that the Ca/P ratio in the food is below 1.0.

Classical Feeder insects as well as supplements are rich in P. So you have to give a pure Ca salt (Ca lactate or Ca citrate) in addition to the standard supplements.

Also check your temps and lights. Make sure you have chill out areas with room temp in the tankl and basking areas which reach 50 000 lux and more.
