I'm going to need to see some more pictures...just kidding! Yes, it could be shell rot. Three-toes are very succeptible to shell rot. With the trauma she's obviously endured, it could have opened her up to an infection. Or, it could just be scarring. You should take her to a good exotic vet that is knowledgeable about turtles.

well i just soaked her in some warm water.. and brushed her whole shell with an old tooth brush...

theres no soft spots/slimey areas.. or odor.

i did rub her down with some nolvasan tho just incase.

I wouldn't be as concerned about her shell, her cloaca is a different story though, why's it swollen and appears to be irritated?

I would see a vet very soon. Very very soon.
I just went through a horrible ordeal babysitting aquatic yearlings with "shell rot."
I have a feeling the little map isn't living now but I'm not sure, haven't heard back from the owner.
This map had an ongoing shell infection for months on end without treatment. I think by the time treatment began it was too late. 2 days before the owner came back from vacation the little map's leg fell off. I've discussed this with an exotics vet who speculates there was likely infection in the bone already because the infection had gone on for too long without treatment.
I tried to tell the owner months ago that you should not allow a shell infection to go on for long without antibiotics but I wasn't heard & now the turtle has suffered because of that.
If treatment had been done months ago it wouldn't have cost very much & this would've cleared up back then and the turtle would've been fine but that didn't happen.

What is Novasan?
Is it an antibiotic topical cream or somthing like that?

Nolvasan is an antiseptic solution that comes in many forms. You can get it in shampoo, as a thick soapy gel, as a watery solution, etc. It is used extensively in the Veterinary and equine fields. Also, it smells wonderful.
