Has he been breeding? Are you sure it isn't one of his hemipenis?

If it is a rectal prolape you need to get to the vet asap.
Rectal would be comeing out of the vent area.

Can you get a pic? someone would be able to tell.

The hemi would be closer tword the bulges to the side a bit. With a hemi prolapse you can soak in thick sugar water and keep him moist(papertowel substrate) and see if it goes back in after a few soaks. If not he will need a vet for that as well.

Keep and eye on your gecko...the fact that there was some blood could mean that he's open to infection. When a prolapse occurs its important to keep the tissue that is sticking out moist or it will become necrotic. If its an intestinal prolapse, then its a medical emergency. If its a hemipene and stays out then it is too.

Here's some information on what to do with a prolapse should it occur again...

Here is a pic of my male after breeding. If it is his hemipenis it still should go back in after a few minutes. Males will lick at it usually until it goes back in. That is what he is doing in the pic.

I know this is a serious post and all but I bet he didn't appreciate you taking that picture of him at that moment. HAHA. I just had to find humor in the fact that if it was a person in a compromised position like that I would definitely not want a picture of myself on the internet. Anyway, sorry I just found it funny.
