it does not happen as often as you may think, but when it does it is excedingly dangerous (not to mention painful) to the leo. that is why you should avoid substrates like bark, gravel, or sand. go with things like slate, tiles, repti-carpet, paper towels, etc etc...things that cannot be eaten.

My leo was picking at the paper towels yesterday! She couldn't actually eat it, but I'm sure she swallowed a few fibers. I saw her coming out of her hide, looking for food. She could smell the juice stains on the paper towels that were left by the other mealies.
Baby gecko0s can get it from sand and even adults if its not fine sand.
