Very strange skin and tissue problem

My son and I have been caring for our veiled chameleon for about three years now. We have gone through some problems with our veiled but he seems to be strong and active.
I have just noticed a very strange and disturbing problem with his skin and tissue covering the parietal bone above his skull. The tissue seems to be dieing and he has developed a hole through the top cavity extending along the top of his skull. There is very little bleeding and it seems to have sealed itself from the skull cavity however the rest of the skin covering the bone has turned a grayish white color and seems to be dieing away. There is no chance this could be shedding issue.

Does anyone have any idea what could be causing this problem? I have just recently replace the uvb light which I suspect was way overdue. We now have a 5% uvb along with the normal 100w heat lamp. We did visit the vet for another strange problem along his spine where he appeared to have damaged a small area. The vet thought it looked like a burn however I don't believe he could come in direct contact with the screen under the heat lamp or get close enough to receive a burn. The area along his spine has healed itself and now this problem has us concerned.

Anyone familiar with this condition or have any suggestions as to the cause?

I think it would be a good idea to visit the vet again.
