From a local glass company.
You tell them the size of your opening, they cut the glass, and if you want it tempered and/or laminated, for safety reasons, they'll send it off to have that done.
In your divider thread below you said glass was expensive but I don't think it is. I replaced an end piece on a 55gal. tank not long ago and the glass was aprox. 13in x 21in x 1/4in thick, for $6. Not treated or anything, though.

Local glass shops like HH said. And it should not be much more expensive than plexi or acrylic, depending on what type of glass you get. When it comes to plexiglass or acrylic you really get what you pay for and I generally recommend avoiding it.
