I can only answer 1 question but from my experience FedEx is defiantly the best at handling live shipments and getting them to the destination on time.

Typically only one heat/cold pack is needed per shipping box... if a large box, may need two..but if the box is big enough to require two heat/cold packs, it will be more feasible to just ship two smaller boxes..as the shipping cost tends to be cheaper for two small boxes than one huge box.
