There's an application and a questionnaire to fill out. You also need to file the critical incident disaster plan part A and keep part B on file at your house.
The cost of microchipping varies from vet to vet. My vet charged me $40 and waived an exam fee so I would activate the chip ($14 through Home Again).
go to http://www.myfwc.com look under licences/permits on left side
once there look for reptiles of concern/venomous licencse
print out all three documents, the questionairre, the application and the disaster plan
you send in a check for 100.00, the questionairre, application, and PART A ONLY for the disaster plan, the rest should be filled out but kept in your records. You may also print out the inventory sheet, you keep that as well for when they inspect.
Any vet will charge 20-40.00 for a microchip
