Has anybody here attempted to purchase their own exotic snake antivenom before? I have the FDA 1751&1752 forms from venomousreptiles.org but still have many questions in how to go about it. I am confused...do I need to fill out a different form for each antivenom I need to buy or just 2 original package inserts from any of the Avs and then contact each manufacturer? I think I know all the manufacturer's I need to contact's email address except for the Thai Red Cross Society's whose website is mostly in Thai....any help is appreciated.

You will also need an APHIS permit from USDA for importation. I think one BB-IND is all you will need, include the inserts for all necessary AV. What you need is similar to that used by zoos, and to the best of my knowledge, they obtain a single BB-IND. The paperwork I use was submitted many years ago and I do not know if times have changed the procedure. Clarifications are best obtained directly from the Blood and Biological section of the FDA.
