It looks like some sort of fungus infection that's eating away at the flesh.
I'd remove him from the water for a few days (he'll be fine out of the water and you want to keep him dry while you treat this) while I used hydrogen peroxide immediately, then looked for some medicine to put on it.
The tank... I'd do a cleaning and add fresh water, etc., then put in a good anti-fungus medicine. The best that I've found is "Fungus Eliminator" by Jungle Laboratories. It's sold for fish but it has always worked for me and has never harmed any of my various kinds of turtles in all the years that I've used it.
My turtle is finally now growing rapidly and its toenails are growing back. You contacted me off forum and weeks or months after I asked for help. I was using other fish store products with the same ingredients as "fungus eliminator." I couldn't find the exact same product, but I got Jungle's "Fungus Clear." It's a gimmicky Alka-Seltzer-like fizzy tablet, but it worked great.
I was feeling pretty bad that I can keep and breed eastern box turtles for nearly ten years while living in a city, but I was having trouble keeping a snapping turtle.
Thanks again for the good advice!
Jamie
