A veterinary dermatology specialist is sometimes your best bet for skin problems. Your veterinarian should be able to refer you to one. For the few hot spots my dogs have gotten, I've found coal tar shampoo from the veterinarian to be helpful. You can even make a paste of it and apply to just the hot spot, leave on for 10 minutes, and rinse thoroughly. But you need to find out the underlying cause of the hot spots and deal with that. They also need to be monitored by a veterinarian, because they can progress to serious systemic infections.
