This is not unusual with Beagles and some other breeds. She needs to be out of the crate until the problem is solved, or it will become a permanent habit. Be sure to have her checked out by your veterinarian, too, since urinary tract infections are common in dogs, particularly in young females.
Instead of a crate, use a small room or portable exercise pen for confinement for awhile. Give her papers or other potty area on one end and bed, water, and toys on the other end. This lets her instincts to keep her bed clean get stronger again. You may be able to crate her again later on after this is resolved. Possibly you'll need the crate to get her through the destructive chewing stage.
