Housebreaking and chewing

I have an 11 month old Jack Russell-Pug mix pup. In a lot of ways, she's a great dog, but we can't get her to stop doing a couple problems.

1 - Housebreaking: she knows she's supposed to go outside and she does. We have a regular schedule with her and our 2 other dogs. But sometimes she'll tell us that she has to go out, and other times it's like she can't be bothered. In the middle of the night, she'll often go on the bedroom floor. Other times, she'll have just gone out and still then go in the house a little later. Mostly, it's a pooing problem, although in the middle of the night, she'll still pee.

On weekends, I try to get her out a lot to avoid this. She's not stupid (on the contrary, she's too smart!). She KNOWS outside is for going to the bathroom.

On the other hand, she's been good in her crate for months -- until the last 3 days. Now she's starting to poo in her crate. Today, my husband let them out, then put them in their crates. He was gone only 2 hours and she had made a mess.

2 - She's a chewer! Two comforters have been torn up, numerous shoes, the corner of the bed etc. No matter the discipline (no, I'm not brutal or beating her), she will not stop. I do get her chews, but she'll chew that and then a shoe or whatever, then go back to the chew. It's why she's in her crate still when we go out.

3 - She only does this with me. She nips -- my hand when we play (she jump past the toy and go for my hand) or, if I'm busy with something and she wants my attention, she jumps up and nips on my leg or arm.

I'm making her sound like a terror. We -are- very frustrated, but I don't think her nature is bad, because she's good with other things. But she has these bad habits.
