bad chronic ear problems

My sister in law's chihuahua has a really bad ear problem. They're smelly, extremely sensitive (he won't let you near them), and you can tell he's uncomfortable. He often nods his head right to left, and when he does, you can actually HEAR something swooshing around in there.

He seems pretty healthy otherwise.

He's been to the vet with the ear problem before, and they clean his ears out, but the gook keeps coming back. It's been like this for years now.

He's one of 7 dogs in a busy household without much time and money. He eats cheap commercial dry dog food, and that's that. It's not very realistic for me to ask her to change his entire diet, but what's the least I could do?

I use something called Blue Power Ear Treatment, but you have to put the liquid in the ears, massage the ears, and let him shake everything out. If the ears are very painful, try putting baby Ambesol gel into the ears to numb them first.
