My Shih Tzu has started running under the blankets all the way to our feet in bed lately. I'm afraid to let her stay there. I'm I being too cautious or can there be a problem with lack of air?

What you might do is get a piece of something that creates an airway and set down in there. A piece of pvc drainpipe would work, and it comes in both square and round. You could color it with a permanent marker if you don't want it white. And you could secure it with a bent wire coat hanger or some other improvised method tucked under the mattress.
I'd also make sure the dog can easily get out of there. Leave at least part of the covers untucked, and something like a ramp or steps there for the dog to climb down. Little dogs wear out their joints jumping off beds.
