Go to the OFA website (www.offa.org). On the first page, a little ways down you'll see the available health surveys for breeds. Choose our and just go through the questions. There is a point early on where you can say this is an update for a dog that's already been reported.
Even if there have been no changes in the health of a dog, you need to go out and report no changes.
This is entirely anonymous! Yes, you are giving an e-mail at the end so that we can check off that you have fulfilled your PHCA responsibility. All actual data is reported with no identifiers - OFA totally disassociates that e-mail from the database.
HINT! Before you fill out the form, read through the survey results. That will give you an idea of what disorders fall into what categories. Honestly, I wish we had even more disorders listed. For example, PPM (persistent pupillary membrane) isn't listed under eye disorders. By itself, it's no big deal, but it can become a big deal if we don't pay attention and breed two dogs with PPM together. My first girl, Pixie, had "stumps" which is a type of PPM, just the strand wasn't attached at both ends. The great eye doc explained how just a few generations that can become something that interferes with vision. Right now, you report that in "other" which doesn't really help us keep track of trends. And I know there's PPM in the breed because you can see it listed on the CERF site (OFA doesn't report conditions, you have to go to CERF) But I digress!

PLEASE, PLEASE fully disclose so that all of us can continue to understand the full helath of our breed and do something about it when necessary!!
