Water monitor toe injuries

My young asian water monitor has lost the front half of both of his front, 2nd to the inside toes. It appears they got injured somehow. I amputated one, and I just noticed the other one dangling from a thread so its going to fall off on its own.

Is this exceptionally common? Any potential causes? Maybe I have a hazard in my cage?

need to describe whats in the cage. any sort of chicken wire or if it hangs from a screen (common when keeping lizards in tanks with a heavy gauge screen cover), or a split/ crack in a log that can get the nails caught. go into detail. do you see bloodstains? or the nail shaft sticking somewhere? -ja

Screen is currently used for one side. But he never tries to climb it. When I say screen I mean very thin mesh (the kind of screen that is only going to work for a baby monitor).
