"cough" in baby eastern

1 year old eastern brought out of brumination 1 month ago along with his sibling. He is a runt (8 cm plastron length). (sibling is fine- twice his size and eating well, etc). He has developed a sound like a mix between cough/sneeze and his entire body recoils when he does it. He extends his neck & opens his mouth a lot and sometimes has a cough & recoil after opening his mouth. He acts like it hurts and withdraws afterward. No nasal or oral discharge. Has eaten earthworms/waxworms only sporadically, lately not at all. He often closes his eyes. Since brumination I have removed caseated pus from his left middle ear twice and treated with mupirocin (ATB) ointment twice x several days.
Aquarium habitat has Zoomed reptisun 5.0 flourscent, 150 watt sunbulb overhead on Zoomed eco earth substrate (high temp 82 degrees F) Any thoughts on helping me with this little guy?

Get this turtle to the vet asap. It could be pnuemonia or some other type of respiratory infection. This turtle needs to be on antibiotics. Raise the temperature slightly and keep a close eye on this little one.
