Help ID Mystery Toad

I need help to ID a toad. It was given away at the local petco as a pacman frog...no were near correct species. I have looked on every toad page I know of, so I am turning to you all for help.

Coconut substrate kept moist, a few water bowls they like to sit in covered by huts and / bowls. A plant or two, Cricket every day or every other day. every 3rd or 4th cricket should be dusted in calcuim. Change substrate every few months... just one frog.....he should be happy. I have had mine for over 4 years....Surprised how hardy they are. Dont sweat it they can survive if you miss a few days feeding, but make sure they have water!
