First (if you have not done so) I would make sure that your tortoise has something that requires treating. You need to know what so that you would know how to treat it. So I would guess a trip to the vet for a wellness exam might be the first order of business.
You can't treat it yourself, because you don't know what kind of worms it has--plus, if it has worms, it might have other parasites as well, and they can all require different meds. I took my marginated tort in today and he was treated with panacur (for the hookworm) and Droncit (turned out he had a couple fluke eggs, too). Just spend the money and get it treated properly by a vet, and you'll be better off in the long run.
