I would wait and just keep it fairly dry and let the snake shed it out on its own. if it has been getting better from shed to shed. and if it does not get better then I would take it to the vet.

I sure would like to know what water blisters are!
It looks like it could be an old injury, especially since hognoses rollover as a defensive mechanism exposing their ventral surface to predators. All that we can say about it is if there's any change for the worse, get it checked out by a vet.
