Pacman frog not doing so well

Okay yesterday I switched my pacman (about 1.5 in.) to a new tank and today I came out and looked in the tank and he was upside-down. So I picked him up and rinsed him off. Now he is still alive but one of his pupils is dialating and the other is contracted and won't move. Also one of his sides is fatter than the other. Anyone have any ideas at all? I know this is a little vague so if you have any questions just ask.

what kind of substrate does your tank have and is it possible that he swallowed it.
