sick pacman, need opinions, long

My adult pacman (thought to be about 1 1/2 years old) has always eaten weekly & soaked in her dish every few days. She didn't eat last week, until I tried again the day after normal feeding day. Then, she only ate 3 crix, when I offered 4. This week she didn't eat at all. She's been sitting in the same spot, not soaking at all & is looking really wrinkley. Last week I washed off her eyes, which had dirt on them, although they were closed. Once I did that, she ate the 3 crix. This week she's again got her eyes closed (no, she's not hibernating, she came out of that about 2 months ago). I took her out & rinsed her off & soaked her & looked at her & it took her a good 5 minutes to get active/fiesty about being messed with. I felt 2 lumps under her chest area, but don't know if that's normal as I never hold her. No noticeable injuries or anything. I replaced her in the water, which she jumped out of & has been sitting in the same place since. She now has one eye closed again & looks just as wrinkley as before I soaked her.
She just looks "off",not as active as normal, not eating & all wrinkley. Any opinions/advice? I'm worried.

When is the last time you changed the substrate??

I took her to the vet on Sat. He thought she's dehydrated & maybe a bit stopped up. He recommended superhydrating her & getting her to poop. I soaked her for an hour that day & she looked much better & ate 3 superworms for me. I soaked her again the next day, & changed her substrate while she soaked. She pooped a little bit.
When I put her back, she buried herself completely in the substrate (which is wet, it's freshly-made coconut fiber) & is chilling there. I plan to unearth her on Wed (weekly feeding day) & try to get her to eat a pinky (vet's advice as she appears thin). I keep the substrate quite dry so she'll use the water to "go", but for some reason she was not soaking herself. I plan to keep the substrate more damp from now on.
I really should have known the signs of dehydration, as I've seen it before in my leos: lethargic, wrinkly, poor appetite...
Thanks all. Hope you learn a bit from my experience, too.
