This is my friend's gecko, I wanted to tell her that her gecko doesn't look right, either underfed or sick, but I'm not too sure. Can a healthy gecko have a fat body and a thin tail?

When my gecko had a fat body and a thin tail he was suffering from an impaction, but might vet told me it could be from parasites. Your friend should get the leo checked by a vet that knows reptiles, or at least get a fecal smaple tested for worms/parasites. I hope he's not sick, good luck!

hm.... It doesn't look too bad to me... Tail could be fatter, and it's a bit dry - could use more moisture.
But if you're truly worried, tell your friend - just be like, "hm, that leo looks kinda weird - how often do you see a leo whose body is bigger than its tail?" Then that might start a conversation, and you can suggest that your friend take it to the vet to make sure it's okay.
