Every once in a while I see a post requesting one of these...this is the method I use to ensure that every animal gets checked & recorded every day.
I have this available in a variety of formats - .jpg, Microsoft Word, Excel, etc., so e-mail me if you'd like a copy & let me know which version.

I created a program a couple weeks ago that runs off of a database that lets you keep record of as many herps as you want. Complete with an "event" section where you may type in daily events, etc. Add pics of each animal to each record.
