Sudden death of Red Eyed Tree frog

Hi -

Wondering if anyone has had this happen. I have lost 2 of my red eyed treefrogs for so known reason. They are acting perfectly normal, eating well have good weight and then I found the laying on the bottom of the tank on their backs dead. This has happened twice. They are in a 20 gallon Tank has a moss bottom, a large piece of driftwood covered in bromeliads they have full spectrum lights over them and are sprayed twice daily.
The moss in over clean topsoil/sand mix with a layer of sand under that for drainage. I am very diligent about removing any droppings or crickets that die before they mold or any minor spot of mold that I might see growing.

I got them all as froglets and they are now close to two inches in length.

Any ideas?? I don't want to lose anymore. I am going to break the tank down and re-establish it. Is there a better set up then what I described above?

do you use water straight from the tap or do you use bottle or water that had set w/ an open lid for 24 hours?
