'Late' Premie?

I just had a 'first' happen to me and I'm hoping someone can offer some insights as to what may have happened, and what I can do in the future to ensure it doesn't happen again...
I had a clutch of T. ibera eggs in the incubator, temp was 86 F - 88 F, humidity was 70%. Two of the eggs pipped and hatched right on schedule. A week later, a third egg pipped but the baby still had a huge yolk sack still attached. Two days after pipping (while still remaining in the egg) the baby died. A week after that (two weeks after the successful hatch) a fourth egg pipped, again with a baby with a huge yolk sack. This baby was dead when I discovered it. The remaining two eggs appear to be infertile.
All the eggs were from the same parents, were incubated under indentical conditions...what went wrong?

I don't think anything went "wrong" this is just how things happen sometimes. I really can't think of anything you could have done. You possibly could have cut that one out of its egg, but you probably would have lost it anyway. all you can do is do your research and hope for the best. hope things go better next clutch.

What was the living condition of them hatchlings when they were hatched/discovered, did you left them in the incubator for a few days or removed them immediatedly to another/different environments?

I left them in the incubator, right where they were, as I always do.
